Supplemental Forms for NEW Students
Foothill Christian Preschool is licensed by the State of California Department of Social Services and must have the following forms on file for each child attending preschool. Signed and completed forms are due a minimum of 3 weeks prior to your child's start date in order for them to attend.
NOTE: The following forms can be typed directly into, so fill out online, save to your computer and attach them in an email to admissions coordinator, Nora Guerra (nguerra@foothillchristian.org) for ease and convenience.

*IMPORTANT:
You will need to make an appointment with your doctor in advance to ensure this form is completed on time. Without this completed form submitted your child cannot attend.
