New Families — 1st Grade Registration
Dear Parents:
Thank you for your interest in Foothill Christian School. Choosing the best school for your child is not an easy task. Perhaps no decision is more important than where your child will attend school and what type of influence to which he or she is exposed. We believe the primary grades provide the foundation upon which future learning depends. Helping your child meet with success is our number one priority.
Below is a checklist of all the forms necessary to start the admission process. Click on the links, type directly into the forms, then print them out and sign them. Once completed submit them to our office along with the registration fees and a copy of your child’s most recent report card and achievement test scores. Once the application is reviewed, qualifying students will be invited to be tested - by appointment only. FYI: Forms are available in hard copies at the school, if needed.)

Checklist - For your application to be complete, it must include the following:
Online Forms for All Students:
• Enrollment Application
• Emergency Form
• Enrollment Agreement
• Permissions Document
• Involvement Survey
• Financial Rate Sheet
Additional Form for First Graders:
• Health Examination Form
New first grade students must also submit copies of the following items along with all other registration forms:
• Copy of Most Recent Report Card
• Copy of Achievement Test Scores
• Immunization Record
• Copy of Birth Certificate
Registration Packets will not be accepted unless all paperwork is submitted along with the registration fees. New Student = $325
