New Families — 6th-8th Grade Registration
Dear Parents:
Thank you for your interest in Foothill Christian Junior High School. Junior High is a critical time for young students. They need to be presented with a wide range of learning opportunities, which are focused not only on developing potential but building character. As you become more acquainted with us, I’m sure you will discover many positive qualities about our school that fit your needs and expectations.
Below is a checklist of all the forms necessary to start the admission process. Click on the links, type directly into the forms, then print them out and sign them. Once completed, submit them to our office along with the registration fees and a copy of your child’s most recent report card and achievement test scores. Once the application is reviewed, qualifying students will be invited to be tested - by appointment only. FYI: Forms are available in hard copies at the school, if needed.

Checklist - For your application to be complete, it must include the following:
Online Forms for All Students:
• Enrollment Application
• Emergency Form
• Enrollment Agreement
• Permissions Document
• Involvement Survey
• Financial Rate Sheet
• Automatic Bank Debit Enrollment
Additional Forms for New Junior High Students:
• Principal/Teacher/Pastor Reference Form*
Two recommendations need to be filled out and mailed to us by a principal, teacher, and/or pastor before being tested.
New elementary grade students must also submit copies of the following items along with all other registration forms:
• Copy of Most Recent Report Card
• Copy of Achievement Test Scores
• Immunization Record
• Copy of Birth Certificate
Registration Packets will not be accepted unless all paperwork is submitted along with the registration fees. New Student = $325
